Where and When?
If you wish to register with the practice please call in to collect a registration pack or download a copy from our Useful Forms section.
You can register any day of the week at reception. As it can take a little while to carry out this process registrations are only done between 10am-5.30pm. Please avoid coming before 10am as this time is needed for emergencies.
You then must attend a new patient medical appointment which will be scheduled by reception once you have returned the completed registration pack. This appointment is a condition of being accepted as a patient. If you fail to attend the appointment and do not cancel beforehand, we cannot register you as a patient at our practice.
What you’ll need
To register you need to complete the registration pack described above and bring your medical card from your previous surgery if you have one. You will also need to bring the following forms of ID:
- photographic ID (such as a passport or driving licence)
- utility bill
- proof of your address (separate from the utility bill)
When you attend for your new patient medical appointment, please bring with you any details of medication you are currently taking. Ideally this should be in the form of a repeat prescription pink slip from your last surgery or at least the medication boxes with printed labels. This is because you may need to order your regular medication before your medical notes arrive and we need to ensure we are prescribing safely for you.
You will also be asked to complete a medical questionnaire (if you have not done so already). This is because it can take a considerable time for us to receive your medical records and ensures we have some up to date information about your health.
